For the health and safety of our students, all students must have a completed health form on file to attend the Program. The health history form is in the online registration system. It is best to complete the health history form online. You do not need a physician to complete the health history form. The deadline for completing the health history form is June 1st. If you register after June 1st, please complete the health history form as soon as possible.
As an alternative, families can also download the health history form and mail it to us. We cannot accept any health history form via mail after June 1st.
